Eustachian tube is a structure connecting the nasopharynx and middle ear cavity. Normally it is closed but it opens when we swallow or yawn. It serves to equalize the middle ear air pressure to the outside. Moreover during diving, elevating or landing of flight, there is also change of pressure. If the tube does not work, the pressure in the ear will be different to that outside. Thus affecting our hearing. It is known to be a predisposing factor to infection of the middle ear.

Ooo.. sebab tu lah bila telinga jadi 'pekak' kita telan air liur, jadi hilang pekak... sebab bila menelan, waktu tu tiub Eustachio terbuka. =) betullah, bila menguap pun telinga menjadi 'lebih mendengar'!

Masalah bermula apabila.... berlaku jangkitan/inflamasi yang menyebabkan dinding tiub Eustachio membengkak, lantas menyempitkan @ menutup lubang tiub Eustachio. Maka, sebab itulah kita jadi kurang pendengaran...

p/s : dulu2 sy ingatkan dalam tiub Eustachio tu ada cecair...hehehehe..

virus: M: paramyxovirus
C: varicella zoster virus ZV

symptoms : M: fever, cough, runny nose, red eyes and a generalized, maculopapular, erythematous rash.

C: in adolescents and adults: anorexia, myalgia, nausea, fever, headache, and malaise

in children: papular rash, followed by devolopment of malaise, fever [a temperature of
100-102F, but may be as high as 106F in rare cases], and anorexia.

Spreading method : M: through respiration (contact with fluids from an infected person's nose and mouth,
either directly or through aerosol transmission), and is highly contagious
C: coughs or sneezes of ill individuals, or through direct contact with secretions from the
rash.

Incubation period : M: average incubation period of 14 days (range 6–19 days)
C: 10 to 21 days after contact with an infected person for someone to develop
chickenpox.

Infectivity period: M: infectivity lasts from 2–4 days prior, until 2–5 days following the onset of the rash

malam tadi tidur cuma 3 jam... nak ganti sekarang.. tapi mata tak mampu berkhayal.. Ngantukks.. >.<

1paper telah diketahui khabarnya... -.-" menarik, rasa macam pergi buat test je hari ni..

Har, nak tau tak, yang menariknya...

hurm.ntah, sy pun tak tahu mengapa..

Tapi, waktu sy mula untuk menjawab exam Paper Surgery pagi tadi, lagu UNIC Langkah Tercipta berkumandang... lalu ia mengiringi sepanjang-panjang 1jam 30minit itu. Tak bisa diusir...seperti ada yang sedang bernyanyi di telinga saya..

Dan petang tadi pula... jam 2petang bila saja sy mula menjawab, serta merta Qasidah Burdah pun bermula. Pelik, sangat pelik. macam sy hafal pula lah..sempat pula nak menyanyi kat telinga, padahal '40 soalan kena banyak fikir' ada depan mata. Berusaha juga agar jangan menyanyi di telinga, fokus...fokus.. hhe. Layan je lah..

My Day

Last nite I'm not really sure at what time I fall asleep. I am pretty sure I read on liver anatomy, biliary system and then quickly cover my face with the notes and act like recalling in my memory, knowing that I'm going to sleep for sure. Hhee. A denial state of being a student again happens.

I just remember that I woke up a few times to see the room's light was still on and quickly covered my face with any pillow and back to dream. But the situation wasn't really helpful for a rest, the brain kept thinking and talking.

'I have to wake up early and be at the hospital by 740am tomorrow.'

'ohh..I'm not ready for tomorrow.'

'who will be presenting a case? will it be me??'

'i haven't enough knowledge on infectious diseases..how i'm goin to clerk the patient'

'i know i need to read.. but i want to sleep..huh'

I kept waking up until finally i decided to switch off the light. It's not too late, but the clock showed 4am already. Not to mention, after I went to the toilet. hihi

Yeaaa..after that i slept comfortably until I heard the phone's alarm ringing.. Ouh, only God knows how is the feeling. If it's not a self-pushing, i won't be in the toilet and take my bath.

And then! The fresh day comes (^^)

Bengkak peparu, Peparu Berair

I'm learning on pulmonary oedema today (bengkak dalam peparu)

edema/oedema : means accumulation of fluid

pulmonary : related to lungs

Bengkak dalam peparu ni boleh berlaku samaada berpunca daripada masalah daripada jantung atau selain dari jantung.

JANTUNG

Fahaman logiknya, apabila jantung tidak dapat mengepam darah dengan efficient (terutamanya jantung sebelah kiri - ia terima darah beroksigen dari peparu), maka darah tadi akan congested (tau x macam traffic congestion - jamp!), maka ini akan membina tekanan yang tinggi dalam salur darah bahagian kiri jantung dan begitu juga dalam salur darah kecil dari peparu -(kalau orang depan jamp, orang belakang pun tentu akan jamp kn kn kn). Therefore, this very high pressure will cause the fluid to leak from the capillaries into the lung. Taraaa... terbentuklah komuniti fluid baru yang abnormal dalam peparu. Maka sudah tentu ini mengganggu proses pertukaran gas di peparu. Anda pun akan mula berasa sesak nafas, tercungap2 etc

SELAIN JANTUNG

Ok, jantung mungkin sahaja normal. Tapi problem yang berlaku sekarang adalah pada tisu-tisu di peparu sendiri. Mungkin lining peparu dah damaged thus causing fluid from capillaries to leak. Gas beracun, jangkitan kuman pada peparu, asap tebal yang disedut dalam kebakaran misalnya, boleh menyebabkan lining tissue ni damaged. Sebab-sebab lain termasuk effect overload dari sesetengah ubat/drug. Jangan lupa, masalah buah pinggang (jangkitan kuman, misalnya acute glomerulonephritis) juga boleh membawa kepada bengkak pada peparu. Kejadian lemas dan apa-apa yang boleh menyumbang kepada pengumpulan air yang banyak dalam badan juga boleh mengakibatkan bengkak pada peparu.

Bengkak peparu ini sekiranya tidak dirawat akhirnya akan mengakibatkan satu kondisi yang lebih serius iaitu paru-paru berair (pleural effusion). Ini lebih serius kerana fluid tadi kini bukan sahaja terkumpul dalam peparu, tetapi telah keluar dari peparu ke kantung yang membalut peparu. Enough accumulation of fluid dalam kantung ini boleh menyebabkan peparu kita yang macam sponge tu collapse (mengakibatkan peparu mengecut) thus we can no longer breathe as normal. Huh?!

So, jangan ambil remeh sekiranya anda merasa tidak cukup nafas tanpa sebab yang kukuh (ie: berfutsal). Segeralah bertemu mereka yang boleh membantu.

Simptom penting:

• sesak nafas
• sesak nafas apabila baring (can't lying down flat)
• Feeling of "air hunger" or "drowning" (if this occurs suddenly, awakening you from sleep and causing you to sit up and catch your breath, it's called "paroxysmal nocturnal dyspnea")
• +/- batuk bersama kahak pink

(bila diputar, nanti besi -tempat pemijak kaki sebenarnya- tu akan bergerak ke atas ke bawah (kiri kanan) dan akan terasa lah seperti berombak hehehe..

Dermatitis

Hand dermatitis is a inflammation condition that causes skin to be reddish, very itchy and scaly generally on the hand. It usually occurs when the hand comes in contact with any substance such as an irritant or that causes allergic reaction. When the hands are affected small blisters may be formed on the sides of the fingers. Cracking and splitting on the palms or around the fingers can cause pain. The hands can become dry. Hand dermatitis can also spread to the forearms and the feet.

The causes of hand dermatitis

• It may be caused by touching an irritant or by allergic reaction to any substance
• Contact with products like solvents like turpentine, kerosene which makes the skin dry
• Genetics can also cause hand dermatitis
• Frequent wetting and also drying of the hands and contact with detergents and soaps can cause irritation of the hands
• Cold weather conditions

The symptoms of hand dermatitis

• Redness and itching of the hands
• Blisters may occur due to bacterial infection
• The skin of the affected area becomes much thicker
• The affected area becomes very sensitive
• Bacterial infection can cause crusts, pain and pustules

To treat hand dermatitis avoid working in wet conditions. Wear vinyl gloves to protect your hands. Mud gloves can be used when working outside. Apply a thick barrier of cream on the affected area. Topical anti-biotic creams can be applied on the affected area to prevent infections.

Basically, The Salter-Harris Classification is classification designed for fractures that involve the growth plate (aka physis). This is where our skeleton grow. Yeah, it grows as we grow ;)

Let learn something about our growing bone. Grow grow GROW!!!

This example is showing the wrist bone (pergelangan tangan) where the physis is commonly injured.

'The growth plate is made up of a special kind of cartilage called (surprise!) growth plate cartilage.'

The epiphysis is the end of the bone, with the cartilage that makes up your joint ("epi" means "upon", so an epiphysis is the part of the bone that is on the physis). The physis is the growth plate, the part of the bone that has the cells that allows the bone to grow longer. The metaphysis is the broad region of bone right next to the physis. The diaphysis is the narrow center part of the bone.

Yeay, today finally i benefit something from the opthalmology posting.

I can see the inner side of those lovely eyes using the direct opthalmoscope^^. This one is better than the whole last week in which i just saw blurring sunset, blurring blood vessels... in hope of looking at the optic disc which i can never imagine how it should be looks like.

But today we are so lucky to have an SLE patient with very lovely normal eyes. Now i can confidently saying that what i'm looking at is the pink optic disc.. the blackish macula..and the yellow spot fovea.

yeah..as big as this.. then wandering along the blood vessels and the rest of eye ;)

Today in the opthalmology ward, in the middle of 'dull' morning ward teaching. i was caught as a 'patient'

Dr. Elizabeth says i am a little bit squint.

They do corneal reflex test on me. They shine a pen torch in front of me and see where's the reflection located. And it's positive. the light doesn't really sit in the centre of cornea of my right eye.

My right eye has a slight convergence squint. Ops

Minggu ini sy posting di bahagian ENT (ear, nose & throat) atau bahasa medikus nya otolaryngology.

Kite belajar cara menggunakan autoskop, sejenis teropong telinga. Maka dengannya kite dapat melihat sejauh gegendang telinga (tympanic membrane @ eardrum). Satu fakta menarik tentang gegendang telinga ialah, apabila disuluh dengan cahaya melalui alat autoskop, akan terlihat satu jalur menyenget halus yang akan memantulkan cahaya -maka ia akan terlihat seolah-olah satu benda bersinar di hujung terowong telinga. Fuh, kadang-kadang bila tak nampak gegendang yang sekeliling tapi hanya nampak cahaya. Macam nampak sebutir bintang dalam telinga pun ada ^^

Jalur bersinar tadi dipanggil 'cone of light'.

Gegendang telinga yang sihat akan kelihatan warnanya seperti pearly white.

Gegendang telinga yang tidak sihat akan tampak whitish... hurmm..

Selain dari memeriksa gegendang telinga, kulit sekitar sepanjang terowong telinga juga diperiksa. Sebarang abnormaliti mungkin menyumbang kepada bunyi dengung, kurang pendengaran, kepala pusing atau kesakitan pada sang telinga!

Hehe, mari lihat ini dia autoskop

dilengkapi dengan kanta pembesar dan sumber cahaya yang akan keluar di muncung tirus itu -tempat di mana ia dimasukkan sedikit ke dalam terowong telinga...

har ini gegendang telinga sihat yang boleh dilihat melalui autoskop, tp gmbar ni sangat cun, selalu sy teropong taklah clear macam ni -.-" tapi nampak kan ada slash of light kat situ^^

har ni logik sikit la.. dari jauh nampak gegendang telinga^^

p/s : eh, sy baru tau takpe jea rupanya kalau air masuk telinga...sebab kita ada gegendang telinga sebagai protection! hehe.. So, semalam dalam bilik fibrotoskopi (btul kut ejaan begini...huh) sy tengok dr buat 'ear toilet'...untuk membersihkan telinga dengan cara memasukkan air ke dalam telinga. So that wax terkumpul yang menganggu proses pendengaran dapat dikeluarkan... Tau kan, tak boleh guna cottonbut. Sebab telinga kita hakikatnya ada mekanisme pembersihannya yang tersendiri. So, tak perlu korek telinga! Bersihkan yang luar-luar jer ya.. Otherwise, anda akan hanya lebih menolak wax itu lebih jauh ke dalam atau menyebabkan ia lagi melekat dengan dinding salur telinga. which is...not good..not good

Aha, yang menarik, kami menjumpai satu keanehan dalam telinga kawan-kawan kami sendiri. Hikhik.. nanti kita bercerita kemudian yea.. Adios~
