Nappy rash or diaper rash (generic term)
are common rashes of infancy. Irritant diaper dermatitis (IDD) is the mostcommon rashes of infancy. Typically characterized by erythematous patchesoccurring mainly in the convex surfaces of the buttocks, perineal region, lowerabdomen and top of thighs. Severe forms are associated with skin denudation/erosions or ulcer formation. The rash occurs as a result of prolonged skin exposure to urine, especially when diapers are changed infrequently. The increase in moisture and acidity as a result of prolonged urine exposure results in the breakdown of stratum corneum. In addition, faecal ureases increases alkalinity of the local environment which acts as a skin irritant during prolonged exposure to faeces or increased stooling frequency(i.e. diarrhea).
The best treatment for IDD is to
discontinue the use of diapers and allow the area to air dry; however, this may not be a convenient/realistic option for most parents. Other treatments include various moisture barriers such as talcum powder (moisture absorbing), zinc oxide ointment, petroleum jelly, etc. In severe cases, mild corticosteroid ointment may be used.
It is not uncommon for secondary infections to occur when a child has IDD. One of the most common secondary infections is candida infection. The rash is usually characterized as erythematous, includes
concave surfaces and may have satellite lesions. This rash is easily treated with antifungal cream, eg. nystatin cream.
